摘 要: 目的: 观察奥卡西平（ OXC） 与卡马西平（ CBZ） 对老年三叉神经痛（ PTN） 患者血浆 P 物质（ SP） 水平及睡眠质量的影响比较。方法: 按随机数表法将 142 例老年 PTN 患者分为 OXC 组（ 研究组） 和 CBZ 组（ 对照组） 各 71 例。于治疗前（ T1） 、治疗 2 个月后（ T2） 评估 2 组患者疼痛程度［简化 McGill 疼痛问卷-2（ SF-MPQ-2） ］和睡眠质量［理查兹-坎贝尔睡眠量表（ ＲCSQ） ］，检测两组患者血浆 P 物质（ SP） 、β-内啡肽（ β-EP） 水平，观察不良反应情况。结果: T2 时，两组 SF-MPQ-2 和ＲCSQ 评分、血浆 SP 水平均较T1 时降低，且研究组低于对照组（ P 均 ＜ 0． 05） ;血浆 β-EP 水平则较 T1 时升高，且研究组高于对照组（ P 均 ＜ 0． 05） 。研究组不良反应发生率低于对照组（ P ＜
0． 05） 。结论: OXC 对老年 PTN 患者疼痛、睡眠改善情况优于 CBZ，且不良反应发生率较 CBZ 低，更具安全性。

Effects of oxcarbazepine ａnd carbamazepine on plasma substance P level ａnd sleep quality in elderly patients with primary trigeminal neuralgia

Abstract: Objective: To observe the effects of oxcarbazepine （ OXC） ａnd carbamazepine （ CBZ） on plasma substance P （ SP） level ａnd sleep quality in elderly patients with primary trigeminal neuralgia （ PTN） ． Methods: 142 cases of elderly PTN patients were divided into OXC group （ study group） ａnd CBZ group （ control group） with 71 cases in each group ac- cording to the random number table method． The pain degree ［short-form McGill Pain Questionnaire-2 （ SF-MPQ-2） ］ ａnd sleep quality ［Ｒichards-Campbell Sleep Questionnaire （ ＲCSQ） ］ were assessed in the two groups before treatment （ T1） ａnd after 2 months of treatment （ T2） ，and the levels of plasma substance P （ SP） ａnd β-endorphin （ β-EP） were measured in the two groups，and adverse reactions were observed． Ｒesults: At T2，the scores of SF-MPQ-2 ａnd ＲCSQ ａnd the plasma SP level in the two groups were lower than those at T1，and the indexes in study group were lower than those in control group （ all P ＜ 0． 05） ． The plasma β-EP level was higher than that at T1，and the level in study group was higher than that in control group （ all P ＜ 0． 05） ． The incidence rate of adverse reactions in study group was lower than that in control group （ P ＜ 0． 05） ． Conclusions: OXC is superior to CBZ in the improvement of pain ａnd sleep in elderly PTN patients，and the incidence rate of adverse reactions is lower than that of CBZ，and OXC has higher safety． Key Words: Oxcarbazepine; Carbamazepine; Trigeminal neuralgia; Sleep quality; Elderly


慢性顽固性三叉神经痛（ TN） 具有反复、剧烈、突发、间歇期症状不显著等特点，原发性 TN（ PTN） 常见于老年人群，发病率约为 4 ～ 5 /10 000。本病引起的剧烈疼痛不仅造成患者痛苦，还对其饮食、睡眠 等造成严重影响。奥卡西平（ OXC ） 、卡 马西平

（ CBZ） 均为治疗 PTN 的一线药物。文献显示，OXC使用时无需常规监测血药浓度，药物相互作用较少［1］，可能较 CBZ 更具优势。对此，本研究观察 OXC、CBZ 在 PTN 中的应用情况，现报告如下。
